You are not doing anything wrong...
It is the nature of the iOS app which sometimes shows your reply under another reply. But, if you go to the browser, you will find that your reply is under the original post. Check this thread out on the browser and you will see what I mean:

Thanks! I think at one point I did cross reference against the browser and while I was not rigorous enough to really check, I think I noticed something was different.
Do you use the browser on your phone or are you not on an iPhone? And therefore it doesn’t matter?
